This form is for existing finance customers only who have recently received a letter from us regarding their current finance product term, which will shortly come to an end.

Important: You should only complete the application form if you are happy to proceed with your rate switch without advice. Please be aware that by switching your product in this way, you will not receive advice.

By completing this form you confirm you are acting on behalf of all parties to the finance agreement and have sought their prior permission.
